Full time Discharge Officer at Newcastle Under Lyme Borough Council

About the job
We are looking to recruit a motivated and driven individual who is passionate about ensuring there are safe and effective discharge arrangements in place for those leaving hospitals or prison settings; who have no fixed abode and will be at risk of rough sleeping or homelessness.
The role will develop and maintain a high level of contact with hospital and prison services in order to develop partnership working for effective and successful outcomes.
In this role you will oversee the timely and efficient discharge of individuals with No Fixed Abode from hospitals and prisons. You will work alongside hospital and prison staff to monitor and progress discharge plans of those identified as having NFA to ensure the right support and arrangements are in place, so individuals are not discharged/released onto the streets.
Requirements
Ideally, you will have experience in this or a similar role and have a sound understanding of the complexities and barriers that may be faced.
The ability to provide a responsive, proactive and creative service to provide safe and effective assessments of individuals before discharge/release and the ability to develop and maintain strong working partnerships. The role is part of our ongoing commitment to reduce rough sleeping and homelessness.
