Job Description

A rare and exciting opportunity has arisen, giving you the opportunity to join our Community Interventions Team to work specifically with Rough Sleepers.

The Department of Levelling Up, Homes & Communities (DLUHC) has provided funding to Middlesbrough Council to deliver on their plan to ensure rough sleeping is prevented wherever possible, and where it does occur it is rare, brief and non-recurrent.

This role is opportunity for a passionate, motivated individual to provide outreach support and manage an intensive case load of individuals who are rough sleeping, or at risk of returning to Rough Sleeping.

As part of the role, you will be required to:

Take part in 3x early morning sweeps of the borough, starting around 5:30am.

Proactively engage with service users at drop-in sessions held by Voluntary Organisations.

Attend The Rough Sleeper Action Group each month

Liaise with a range of accommodation providers to engage with service users at risk of returning to rough sleeping.

This is an exciting time for Middlesbrough as we develop ACT (Accessing Change Together) which integrates council staff and partner organisations and provides a range of support and interventions in relation to service users who need support with Housing Support and Homelessness, Domestic Abuse and Substance misuse.

ACT (Accessing Change Together) is an integrated service, which means that lots of support services are working together and using a shared case load management system. This will make it easier for service users to get access to services they need so they receive the right support and easier for council staff and partner organisations to work together and share information to improve outcomes for service users. It is recognised that sometimes service users need help in a number of support areas.

Working with a multi-disciplinary approach, the teams aim to react quickly to those needs, developing risk assessments and support plans to address those needs appropriately. The team will work alongside other services including Adult & Children’s Social Care and Public Health, to develop consistent pathways and effective processes.
