37 Monday – Friday, normal working hours
This opening assignment is for 3 months
City: SOUTH SHIELDS
Office based, with the Integrated Hospital Discharge Team, and will cover Monday to Friday, however there may be times when shift work is required, including weekend and Bank Holidays
Enhanced DBS disclosure required
The role of Hospital Discharge Coordinator is to support operational social care teams with ensuring people return home from hospital in a safe and timely manner, through successful partnership working with Health and Social Care. The role will take a proactive coordination approach covering all discharge pathways. There is a requirement of this role to carry out duties in the most flexible, effective, and efficient manner taking a home first approach
